Italian (Apulia, Campania): apparently from ventola ‘fire fan’ perhaps applied as a metonymic occupational name for someone whose work involved making or using such an item or as a nickname for someone with exceptionally large ears or hands.
However, the term is not attested before the 18th century which would be very late for surname formation.
