About this project This collection contains records of indigent individuals who were treated by a physician in Savannah, Georgia from 1887-1896. The collection lists the individuals name, diagnosis, place treated, date, and race. Also included are the minutes for the Board of Health from 1822-1864. These minutes contain information about interments, nuisances, resolutions and various reports from the wards in Savannah.

Project Instructions In this project we will be keying two different form types both having to do with the board of health in Savannah, Georgia from 1822-1896.

For the Sick Register form type, name, date and race will be keyed.

For the Board Minutes form type, name, burial date, birth place and age will be keyed. Please note that not all images that look like Board Minutes form types have information to be keyed. Please mark images without keyable information as the "Cover page, Section header, etc" form type.

NOTE: Board Minutes forms are in handwritten, mostly prose format. Please be aware that Interment and Death information may be located anywhere on the image. Some images contain more than one section with names to be keyed. See sample images in the wiki for clarification.

Board Minutes



Choose the board minutes form type when the image contains the wording "Report of Interments" or "Deaths in the city" with a list of individuals underneath. These images will mostly include an age, place of birth and an interment date.

Prefix

Key any titles before the given name, such as "Dr," "Mr," or "Mrs," of the primary person as seen on the record using the dictionary provided for assistance. Only prefix values should be keyed into the prefix field. Name is usually found after the interment or death date.

Given

Key the first name or initial and any middle names of the primary person as seen on the record using the dictionary provided for assistance. Name is usually found after the interment or death date.

Surname

Key the surname as seen on the record using the dictionary provided for assistance. Name is usually found after the interment or death date.

Suffix

Key all titles, such as "Jr" or "III", following the surname of the primary person as seen on the record using the dictionary provided for assistance. Name is usually found after the interment or death date.

Age at Burial

Key the burial age as seen on the record. Burial age may be found when keying interments. Valid ages include numeric digits between "0" and "120" and fractions between "1/12" and "11/12". If an age includes years, months, and/or days key only the years. For example, if an age appears as 10 years, 7 months, key age as "10". If an age appears in months, such as 10 months, key age as a fraction: "10/12". If an Age includes years plus a fraction, such as 3 3/12, key only the year: "3". If the age is less than one month, key "0".

Birth Place

Key the birth place as seen on the record using the dictionary provided for assistance. If present, birth place is typically located on the end (far right) of a list.

Age at Death

Key the death age as seen on the record. Death age may be found when keying deaths. Valid ages include numeric digits between "0" and "120" and fractions between "1/12" and "11/12". If an age includes years, months, and/or days key only the years. For example, if an age appears as 10 years, 7 months, key age as "10". If an age appears in months, such as 10 months, key age as a fraction: "10/12". If an Age includes years plus a fraction, such as 3 3/12, key only the year: "3". If the age is less than one month, key "0".

Burial Day

Key the day from the burial date as seen on the record.

Burial Month

Key the month in its three-letter abbreviation from the burial date using the dictionary provided to assist you. If the month appears as a number, key as seen.

Burial Year

Key the year from the burial date as seen on the record.

Death Day

Key the day from the death date as seen on the record.

Death Month

Key the month in its three-letter abbreviation from the death date using the dictionary provided to assist you. If the month appears as a number, key as seen.

Death Year

Key the year from the death date as seen on the record.

Sick Register



Choose the sick register form type when the top of the image has the heading "Register of Indigent Sick Treated By City Physician".

Prefix

Key any titles before the given name, such as "Dr," "Mr," or "Mrs," of the primary person as seen on the record using the dictionary provided for assistance. Only prefix values should be keyed into the prefix field. Name may be found in the column labeled "Name" and will be in [given-surname] format.

Given

Key the day from the event date as seen on the record. Event date may be found in the column labeled "Date". On forms where the column "Date" is not present, event date may be found in the column labeled "No".

Event Month

Key the month in its three-letter abbreviation from the event date using the dictionary provided to assist you. If the month appears as a number, key as seen. Event date may be found in the column labeled "Date". On forms where the column "Date" is not present, event date may be found in the column labeled "No".

Event Year

Key the year from the event date as seen on the record when present. Event date may be found in the column labeled "Date". On forms where the column "Date" is not present, event date may be found in the column labeled "No". On some forms event year will appear in the middle of the document. See wiki image samples for clarification.

Choose the "Microfilm" form type for all images that contain information about the microfilm reel. These are images that were introduced to the collection when it was microfilmed, and are not part of the registers. See the wiki page for examples.

Cover page, Section header, etc” type for images that don’t contain any data, but might be interesting to look at because they provide some type of context for the image set. For instance, historical notes, affidavits, and so forth.
