The Admission form type can be identified by having the words "Admitted" or "Admission" at the top of the image.

Prefix

Key any titles before the Given name, such as "Dr", "Rev", "Mr" or "Mrs", using the dictionary provided for assistance. If a prefix does not appear in the dictionary then key the prefix as seen. Only prefix values should go in the prefix field.When provided, the Prefix can be located before the Given Name.

Key the Sex or Gender into the Gender field from the column labled Sex or Gender.The location of the Gender can be in the header of the image or my the Given name. If Gender is located at the top of the image then copy down for each record.

When provided, key the Day from the Birth date which can be located by the title of: "Date of Birth".

Key numbers for months where the value is a number and if the month is written out, key as a three character abbreviation based on the dictionary. If not in the dictionary, key as seen.When provided, Birth Date can be located by the title: "Date of Birth".

Key the two or four digit number designated as the birth year in the birth date. The Year must be either two or four digits.When provided, Birth Date can be located by the title: "Date of Birth".

The Commitment form type can be identified with the phrase: "Conduct during Detention" and will have a Committed or a Conviction date that will be keyed as the Event Date.Be aware that the Commitment will also list the word "Admitted" at the top of the image, but the Commitment will have a Committed date or a Conviction date that will be keyed as the Event Date.
